Select Initial Evaluation Required for All New Patients to begin psychiatric care, or select Free 15-Minute Consultation for a brief fit consultation.

Choose an available appointment. Confirm that the state where you will be physically located during the visit and the displayed time zone are correct.

Confirm Your Appointment

Review the appointment type, provider, date, time, duration, and time zone. Select CONFIRM APPOINTMENT to complete your booking. A confirmation email will be sent after the appointment is scheduled.

Before Your Appointment

  • Complete all required intake paperwork before your appointment.
  • Join the telehealth visit from a private and quiet location.
  • Have a valid photo ID and current medication list available.
  • Confirm that you will be physically located in an eligible state at the time of your appointment.
  • Eligible appointments may be viewed, rescheduled, or canceled through the Patient Portal in accordance with practice policies.
